First Visit: In-Person Requirement

Please note that your first visit must be in-person to ensure a proper introduction and thorough assessment. This allows us to build a strong foundation for your care and address any concerns you may have.

Patient Portal and Communication

Upon scheduling your first appointment, our staff will contact you to complete your charts and send a new patient package through your patient portal. It is important to mark these emails as safe to ensure you receive future notifications about appointments and information sent to your portal. We kindly request that you acknowledge messages received through the portal system, which helps us keep track of documents and messages.

Appointment Confirmation

We ask all patients to confirm their attendance for appointments by following instructions provided in reminder messages sent via phone or email. This supports our clinic’s workflow and ensures appointment times remain on track.

Transferring Your Medical Records

If you wish to transfer your medical records to or from our clinic, we require a signed consent form. Please note that there is a fee associated with this service, which is dependent on the size of your medical record and the time it takes to summarize and collate the information.

Please note that all form completions and specialist referrals require an assessment during an appointment. This ensures that we have a comprehensive understanding of your medical needs and can provide personalized care.

Cancellation Policy

  • To maintain our commitment to short wait times, we require a 24-hour notice for cancellations. Failure to provide adequate notice or not attending your appointment may result in a $65.00 fee.

Test Results

  • We prioritize discussing test results in person to address any questions or concerns you may have. To review your x-ray, ultrasound, or other diagnostic test results, please book an appointment with your primary care provider.

Prescription Repeat Requests

  • For optimal medical care, we recommend scheduling an appointment to review your medical conditions and medications. This ensures proper management and allows us to address any concerns.

Electronic Prescriptions

  • We offer the convenience of electronic prescriptions, which can be sent directly to your preferred pharmacy for medication dispensing during your consultation. This is a time-saving alternative to traditional paper prescriptions.
